What Research Suggests
A randomized, double-blind clinical trial in people with dry mouth found that a detergent-free toothpaste containing 4% betaine significantly improved measures including lip dryness and difficulty eating.
The betaine toothpaste relieved dry-mouth symptoms in 44% of participants with xerostomia, compared with 7% using the reference toothpaste.

An earlier double-blind crossover study found that toothpaste containing 4% betaine significantly relieved several subjective dry-mouth symptoms.
The study did not find significant changes in plaque or oral levels of mutans streptococci, lactobacilli, or Candida, supporting betaine’s role primarily as a moisture- and comfort-focused ingredient rather than an antimicrobial one.

A randomized controlled trial in 51 people compared a betaine-based mouthwash, a malic-acid spray, and placebo. All groups improved in dry-mouth sensation and unstimulated salivary flow, so betaine did not separate from placebo on those measures. However, oral-health-related quality of life improved significantly with the betaine-containing mouthwash.

A clinical study of topical dry-mouth products containing betaine, xylitol, and olive oil found increased unstimulated salivary flow, fewer dry-mouth complaints, and improved dry-mouth-related quality of life.
Because several ingredients were used together, betaine’s individual contribution cannot be separated from the overall formulation.
